The most useful worksheet is not simply a list of dollar amounts. Give every number six fields: the benefit being described, the amount or percentage, the source, the date and time, the reference number or representative name, and its status. Use only three status labels: confirmed, needs review, or not established. This prevents a quick phone estimate from becoming a presumed guarantee.

A plan may reimburse part of an eligible service while leaving you responsible for a deductible, coinsurance, charges above the plan's allowed amount, or services the plan excludes. Private pay can mean paying all charges yourself or paying a portion that another source does not cover. Either way, the useful number is not simply a daily rate or a single total. You need to know the billing unit, anticipated duration used for the estimate, deposits, payment dates, likely separate charges, refund rules, and events that can change the amount. Ask for those points in writing before sending money or authorizing a card charge.

Do not begin by asking which total is lower. First ask whether the totals describe the same dates, services, assumptions, and payment responsibilities. An estimate that includes several categories may look more expensive than a shorter estimate that leaves those categories unstated. A blank line is not a zero-dollar charge, and a quoted total is not proof that every clinically appropriate service is included.

A facility may still need to review the person’s circumstances, determine whether its setting can meet those needs, explain financial arrangements, and complete other steps. Likewise, a statement such as “we may have space this week” is not a promise. Ask what has actually been confirmed, what remains under review, and when you should check again.

Start with a three-column note. In the first column, copy the facility's exact claim without improving or interpreting it. In the second, write the source offered, such as a California record, an accreditation directory, a written benefit response, or an explanation from an identified admissions representative. In the third, assign a status: confirmed when a relevant source supports the current claim, needs review when evidence is incomplete or old, and not established when no suitable evidence is provided. Add the date, representative's name, and any follow-up promised.

A statement such as “80% success” may sound precise while leaving out every fact needed to interpret it. Did success mean completing a program, not using heroin for a period, reducing use, remaining in treatment, or reporting improved health? Was the denominator everyone admitted, only people who completed, or only people who answered a survey? Those choices can produce very different percentages.
